Professional Coordination
A Care Coordinator is a geriatric professional with a background in social work, nursing or mental health. Our Life Care Planning clients meet with our Geriatric Care Coordinator immediately after we start working together and makes a complete assessment of the client’s specific needs. Then, she creates the plan that fills those needs. She monitors your loved one’s care closely and makes or recommends appropriate changes to the care plan as needed. She also serves as the front line advocate seeing to it that your loved one always receives the best care regardless of whether it is at home, in an assisted living facility or in a nursing home.
Our Geriatric Care Coordinator is also an expert who knows our local nursing homes, assisted living and other care facilities, and will advise you on the quality, availability and costs of long term care in Riverside and surrounding areas.
